To bid on on our projects, subcontractors must fully complete the screening forms and qualify.
Click these links to download, fill, and return the Annual Pre-qualification Form, insurance requirements and W-9. Send to danam@marchassociates.com.
Kindly complete all applicable forms in their entirety, including all supporting insurance documents listed on page three of the form (including endorsements) and a completed W-9.
If your company works in New York, please pay specific attention to the requirements listed under the New York section of page three and complete the New York Insurance Addendum.
